Please fill in this health questionnaire on the day of the donation or one day beforehand. You can fill in the survey anywhere: at home or for example on your way to the donation venue.

Submitted forms are stored in our system today and tomorrow.

We assess your eligibility to donate blood based on your answers to the health questionnaire and an interview at the donor session. The purpose of the health assessment is to ensure the safety of you and the patient receiving your blood.

Thank you for your interest in donating blood! You cannot donate blood if you are under the age of 18. You may donate blood as soon as you have turned 18.

Please note that first time donor must be under 60 years.

Please note that you can donate blood if you have donated less than two years ago.

Thank you for your interest in donating blood! You cannot donate blood if you are 71 years or older. Blood donation is possible in Finland between the ages 18 and 70.
