Job summary
We have an exciting and unique opportunity for a Band 6 Mental Health Practitioner to work within one of the Bolton integrated neighbourhood teams. These teams include professionals from health, social care and the community and voluntary sector with the aim of improving health and wellbeing in the local community and empowering local residents.
As a Mental Health Practitioner you will provide the mental health expertise within the integrated neighbourhood team. This will require an ability to work flexibly Monday - Friday between 9:00am and 5:00pm.
The post holder will be based in one of the following location in Bolton:
Waters Meeting Health Centre, Pikes Lane Health Centre or Winnifred Kettle Health Centre.
Main duties of the job
To work as part of the integrated neighbourhood team providing support, advice, consultation and where appropriate care for people, regardless of age and diagnosis, presenting with mental health related problems, to enable safe management of clients within Primary Care.

Job responsibilities
- To complete patient assessments as required for people with low level mental health needs such as, but not limited to, low mood/depression, anxiety disorders, mild cognitive impairment and contribute to the assessment of and differentiate between delirium and mental disorder.
- As part of the integrated neighbourhood team, act as a key worker for clients (with a mental health component) who are at risk of hospital admissions due to a long term physical condition.
- To undertake comprehensive assessment including EMI assessments for people with complex health and social care presentations.
- To in reach and provide appropriate clinical support and advise into the intermediate care bed bases in Bolton
- To work as a link clinician; developing professional relationships within Primary Care and Social Care Services to allow effective development of care pathways within localities.
- To triage/consider individuals with complex presentations and to direct them to or refer into specialist services within Primary/Secondary Care or the voluntary/third sector.
- To maintain close working relationships with Secondary Care Services, keeping them informed of developments within Primary Care.
- To have an up-to-date and comprehensive knowledge of local resources and services available and how to access them; providing information and guidance to GPs and other colleagues in Primary Care.
- To consider early intervention principles and provide guidance regarding people with more complex presentations identified in consultation with GPs.
- To provide informal and formal training to GPs and other Primary Care colleagues on topics relating to mental health, in order to develop their skills in the diagnosis and management of mental disorders.
