This is an exciting opportunity for a Band 6 Care coordinator (RMN, Social Worker, Occupational Therapist) to work within a specialist service for people experiencing a First Episode of Psychosis. Age range 14-65.
Main duties of the job
Reduce the stigma associated with psychosis.
Improve professional and lay awareness of the symptoms of psychosis and the need for early assessment.
Reduce the length of time young people remain undiagnosed and untreated.
Develop meaningful engagement, provide evidence based interventions and promote recovery during the early stage of illness.
Increase stability in the lives of service users, facilitate development and personal fulfilment.
Provide a user-led service, i.e. a seamless service available for those from age 14 to 65 years (on acceptance) that effectively integrates child, adolescent and adult mental health services and works in partnership with primary care, education, social services, youth and other services.

To hold and manage a caseload of service users who have been identified as experiencing their first episode of psychosis
To be aware of and adhere to responsibilities for people under CPA, including offering/arranging carers support.
To be aware of and adhere to the NICE guidelines for first episode psychosis.
To manage discharges from the service and transfers to other services as appropriate for service users who have reached the end of their time with the service.
To work as part of our integrated MDT, and to participate in a clinical decision making process to determine the most appropriate management plans for all service users who are under the care of the team. To participate in the duty rota operated by the Early Intervention Team.
