The following form asks for information about you and your scheduling preferences for one of the scheduled focus groups.  Once a focus group has been assigned, you will be sent a Consent Form, which we will ask you to sign and return before the focus group commences.  Please note that the focus groups will all be online (Teams) and may be recorded, but will be anonymised.
