Malarial retinopathy and neurovascular injury in paediatric cerebral malaria

Background
Diseases of the brain are difficult to study because this organ is relatively inaccessible. Only one part of the central nervous system is available to direct, non-invasive observation – the retina. The concept of the retina as a window to the brain has created much interest in the retina as a source of potential markers of brain disease. Paediatric cerebral malaria is a severe neurological complication of infection with the parasite Plasmodium falciparum, which is responsible for death and disability in a significant number of children in sub-Saharan Africa. As with many neurological diseases, the precise mechanisms by which this infection causes damage to the brain remain unclear, and this hampers efforts to develop effective treatments. It may be that studying the retina in paediatric cerebral malaria could both illuminate pathogenesis specific to this disease, and also provide an illustration of how to approach retinal biomarkers in a new, and potentially more effective way.
Methods
I approached the aim of developing retinal features as markers of brain disease in paediatric cerebral malaria via several objectives. I made use of an existing clinical study to collect new retinal data from ophthalmoscopic examinations and fundus fluorescein angiograms from patients over three successive malaria seasons in Malawi, and added these to historical data obtained previously at the same site. I devised a new method for grading retinal images. I reviewed the biological plausibility of associations between retina and brain in cerebral malaria, and then considered analytical methods to interpret my retinal data effectively. Finally I estimated associations between retinal features, outcomes, and a radiological measure of brain swelling using combinations of regression models.
Results
My review of retinal and cerebral histopathology, vascular anatomy and physiology indicated that certain retinal and brain regions may be similarly prone to damage from sequestration as a result of interactions between aberrant rheology and microvascular geometry, such as branching patterns and arteriole to venule ratios. My review of evaluations of analogy and surrogacy suggested that biological similarities between retina and brain could be used to justify statistical evaluation of the amount of information the subject and object of the inference share about a common outcome, as used to assess surrogate end points for clinical trials. This kind of approach is able to address questions about whether a particular retinal feature is effectively equivalent to an analogous disease manifestation in the brain. I report analyses on three overlapping groups of subjects, all of whom had retinopathy positive cerebral malaria: children with admission ophthalmoscopy (n=817), children with admission fluorescein angiography (n=260), and children with admission angiography and MRI of the brain (n=134). Several retinal features are associated with death and longer time to recover consciousness in paediatric cerebral malaria. Broadly speaking, these features appear to reflect two processes: neurovascular sequestration (e.g. orange vessel discolouration and death), and neurovascular leakage (e.g. >5 sites of punctate leak and death). Respective adjusted odds ratios and 95% confidence intervals for these particular associations are: 2.88 (1.64-5.05); and 6.90 (1.52-31.3). Other related processes may also be important, such as ischaemia, which can be extensive. Associations between retina and brain are less clear, in part because of selection bias in the samples.
Conclusions
Neurovascular leak is important in fatal paediatric cerebral malaria, suggesting that fatal brain swelling may occur primarily as a result of vasogenic oedema. Other processes are also likely to be involved, particularly neurovascular sequestration, which is visible on retinal imaging as orange vessels or intravascular filling defects. Sequestration may plausibly cause leak through direct damage to tight junctions and by increasing transmural pressure secondary to venous congestion. Several types of retinal leakage are seen and some of these may represent re-perfusion rather than acute injury. Future work to investigate temporal changes in retinal signs may find clearer associations with radiological and clinical outcomes. The steps taken to evaluate retinal markers in cerebral malaria illustrate a more rigorous approach to retinal biomarkers in general, which can be applied to other neurological diseases

An investigation Into the genetic variation of hepatitis C virus in patients coinfected with HIV

Co-infection with HIV in patients with chronic HCV infection is a common occurrence, which accelerates disease progression and the rate of liver fibrosis. Current literature report conflicting results on the effect of HIV co-infection on HCV at the genetic level. This thesis sets out to further explore the genetic variation of HCV in this group of patients, and use new techniques which may impact upon the diagnosis and management of HCV.
In resource-limited settings such as sub-Saharan Africa, there is a paucity of data concerning rates of active HCV infection in patients with HIV infection. The limited infrastructure and financial restrictions are contributing factors in this. Consequently, cheaper and novel diagnostic procedures are required to promote testing in these important cohorts. A real-time PCR assay for use with pooled plasma and dried plasma spot specimens was developed to screen a large cohort of HIV-infected individuals from Ghana and assess its suitability for screening in such a setting. The prevalence of active HCV infection in this cohort was similar to previous estimates from blood donors but was much lower than estimates from serological tests, highlighting the potential risks in relying on these tests alone in this region. The diversity of strains found within this population agreed well with previous studies.
As the yield of HCV infections was low in Ghana, further studies were completed with the UK and European cohort. A deep sequencing approach was utilised to two effects. The first focussed on a specific notable polymorphism – Q80K in the NS3 gene – to determine whether deep sequencing would benefit the clinic in the detection of this polymorphism at low frequencies, which are below the threshold of detection by traditional population sequencing. The second use of deep sequencing aimed to determine the impact of HIV co-infection on the presence of resistance associated mutations occurring at baseline, studying the largest cohort to date.
The Q80K polymorphism was not observed in any sample as a minority variant, suggesting that the current clinical procedures for pre-treatment screening are suitable and provide sufficient sensitivity. Overall, resistance mutations were relatively common among patients and it was observed that, generally, there were no differences in the prevalences of resistance mutations between HCV mono-infected and HIV/HCV co-infected patients. This finding is in agreement with previous small-scale studies.
This work has direct clinical implications on the use of deep sequencing for screening patients for the presence of resistance mutations. Furthermore, it suggests that co-infected individuals are not at risk of further complications for the treatment of HCV.

Molybdenum-containing oxidases and their application in cascade synthesis

Molybdenum-dependent xanthine oxidoreducatases (XOR) are a family of well characterized enzymes which are known to oxidise purines, imines and aldehydes in cellular metabolism. Despite significant studies relating to drug metabolism, application to chemical synthesis is relatively unexplored.. Herein we report the first use of XORs in the cascade synthesis of carboxylic acids from activated and unactivated alcohols by the combination of the mutant alcohol oxidase GOaseM3-5 and XOR (E.coli XDH & PaoABC). Twenty five carboxylic acids were obtained in quantitative conversion and the methodology compares very well with state-of-the-art catalytic chemical oxidation methods. The reactions were performed in water at ambient temperature and pH using oxygen as the terminal oxidant with the only by-product being H2O2. The oxidation system was also applied to the synthesis of the biomass-derived platform chemical FDCA starting from HMF. The biocatalysts used were able to tolerate a substrate concentration of 100 mM, 20 times higher than previous reported methods with 74% isolated yield of FDCA. Although XORs exhibited a wide substrate scope, no chiral selectivity could be demonstrated. The application of XOR enzymes was also demonstrated in the synthesis of lactams from amino alcohols via an in situ generated imine intermediate. Although pyrrolidone was obtained in 85% yield from its corresponding amino alcohol, longer chain amino alcohols were not well tolerated with either poor or no conversion observed. Although currently limited in substrate scope, this novel biomimetic oxidative cyclisation represents an interesting synthetic approach to lactams, found in a wide variety of biologically interesting target molecules.

Recruitment to randomised controlled trials with children

Recruitment to randomised controlled trials is known to be difficult. Poor recruitment has several adverse consequences. It affects the validity of study findings, is a common cause of trial extensions and may result in premature termination of trials, which is a huge loss in terms of invested funds, resources and lost knowledge. Non-completion or delayed completion of studies maintains the uncertainty about the efficacy and safety of interventions, thereby delaying or preventing the use of effective interventions and prolonging the use of ineffective or potentially harmful treatments. Recruitment of children to randomised controlled trials is thought to be more challenging due to the vulnerability of the population and the fact that consent is provided by another person usually parents. This thesis aims to review the recruitment performance, i.e. comparison of achieved to anticipated recruitment, of randomised controlled trials with children and identify the factors associated with good or poor recruitment.
We undertook a pilot systematic review of recruitment and retention in randomised controlled trials with children, in published literature, and found that few studies report recruitment information but those that do, report very high rates of percentage total recruitment achieved (%TR) and consent. It was not possible to obtain unbiased estimates of recruitment performance and consent rate due to the likelihood of selective reporting and/or non-publication of trials with unsuccessful recruitment.
We subsequently conducted a review of recruitment of children to randomised controlled trials in the National Institute of Health Research (NIHR) Clinical Research Network (CRN) portfolio and found that under-recruitment and delayed recruitment are common problems in paediatric trials. Having a trial manager or coordinator was found to be significantly associated with successful recruitment. Other factors such as being an IMP (Investigational Medicinal Product) vs. non-IMP trial, trial of acute vs. chronic illness, having CTU (Clinical Trials Unit) involvement, pilot/feasibility study and additional trial demands had no statistically significant association with recruitment success.
Since recruitment to a clinical trial can be affected by a number of internal and external factors, we conducted a survey with the clinical teams of a multi-centre randomised controlled trial with children, the MAGNETIC trial, to understand the various facilitators and barriers to recruitment. In order to identify the facilitators and barriers to recruitment and establish the recruitment experience of clinical teams in a systematic manner, we developed an evidence based recruitment survey tool. The survey tool is an online questionnaire that presents a comprehensive evidence based list of facilitators and barriers and free text space for responders to record the strategies applied to overcome these barriers and suggestions for change in organisation of trials to boost recruitment.
The survey of clinical teams recruiting to the MAGNETIC trial found that a motivated clinical team with effective communication skills, effective coordination between study team members at site and between sites and CTU, trial management support, research experience of PI, presence of a research nurse and availability of a designated research team were imperative for trial recruitment success. Heavy clinical workload, shift patterns of work, lack of trained staff particularly out of hours, GCP training, local clinical arrangements and parental anxiety about the safety of experimental treatment were recognised as important barriers to recruitment. A trial specific barrier was difficulty faced by the clinicians in seeking consent from the parents of an acutely ill child in the emergency setting and suggestions were made for consideration of deferred consent.
We concluded that recruitment to randomised controlled trials with children is challenging and poor recruitment and recruitment delays are a common problem. Reporting of recruitment and consent in paediatric trials is poor and needs improvement. Presence of a dedicated trial manager is significantly associated with successful recruitment and the various generic and trial specific facilitators and barriers to recruitment that have been identified can be used by trialists in planning and conducting future clinical trials with children.

"I Just Hope this isn't the End": an interpretative phenomenological analysis of unemployment in university graduates with common mental health issues

The purpose of this thesis is to conduct an exploratory investigation into what it is like to be an unemployed university graduate with common mental health issues (i.e., depression and anxiety) from a qualitative and phenomenological perspective using Interpretative Phenomenological Analysis. Semi-structured interviews with 12 Bachelor's degree graduates in their twenties, who have common mental health issues and have been unemployed between five and 24 months, led to the development of the three master themes of Fall from Grace, Vulnerability and Angst. These findings are explored using a range of theoretical perspectives to demonstrate the devastating impact that unemployment has on these graduates. It constitutes a developmental-existential life-crisis where they are unable to complete their rite of passage into social adulthood, leaving them vulnerable to becoming entrenched in their mental health-related difficulties and less able to overcome their unemployment. The practical implications of this work and suggestions for further research are also discussed.

Assessing the social sustainability of Chinese urban neigbourhoods: a case study of Shenzhen

There is an increasing concern arising from China’s recent urban growth in terms of sustainability from environmental, economic and social perspectives. Huge numbers of new Chinese urban neighbourhoods have been built during the past 20 years with great spatial and social changes in this process compared with the past. However, little attention has been paid to assess the planning and development of these new neighbourhoods against social sustainable criterion. This research aims to evaluate the social sustainability of new Chinese neighbourhoods and discuss its association with patterns of urban form and related planning processes. A new conceptual framework of social sustainability was developed from the relevant sustainable research based upon the Western experiences and adapted to the Chinese urban context. It included three layers, resident’s basic needs, inner social networks and the entire community development for which detailed indicators could be generated as the basis for empirical social sustainability assessments.
An embedded case study methodology was applied to Shenzhen, a new megacity rising from China’s rapid urbanisation process. The methodology consisted of two-phase case studies and utilised multiple survey methods. The city-level focused on understanding the macro-context policies related to neighbourhood development process and the general spatial characteristics of urban form, in which typical patterns could be identified for neighbourhood case selection. Then neighbourhood-level case studies in the Houhai-Dengliang area were used to appraise their levels of social sustainability. The analysis suggests that there is nexus between urban form and social sustainability, with the recently emerging highly-segmented and over-intensified developments being the least socially sustainable form. Defects in the current neighbourhood development mechanisms are further identified based on the clear evidence. The research recommends that the ‘site scale’ and ‘density’ needs to be effectively controlled for new neighbourhoods, within medium ranges, if the best level of social sustainability are to be achieved. For many existing neighbourhoods, evidence also indicates that adopting a collaborative planning approach can be a pathway towards a more socially sustainable development.

An investigation of novel biomarkers of gastric mucosal preneoplasia

(i) Novel biomarkers of gastric preneoplasia following infection with Helicobacter pylori
Despite steady decline in incidence, gastric cancer remains a major global cause of morbidity and mortality and is responsible for over 700,000 deaths worldwide per annum. The commonest form arises following infection with the bacterium Helicobacter pylori. For reasons which remain poorly understood, a small proportion of infected individuals develop gastric epithelial remodelling which follows a well-defined sequence of changes culminating in cancer development. Our group and others have described the role of various gastric mucosal proteins in carcinogenesis following H. pylori infection. We hypothesised that these proteins might be upregulated in gastric preneoplasia and might in turn be used as biomarkers of the same.
We recruited patients attending hospital for diagnostic upper gastrointestinal endoscopy. Gastric biopsies were taken to determine the presence and extent of preneoplastic lesions by histology; H. pylori status; and the abundance of mRNA for putative biomarkers by real-time polymerase chain reaction. Blood was drawn to determine the serum concentrations of H. pylori IgG antibodies, fasting gastrin and pepsinogens 1 and 2. Genomic DNA was extracted from blood samples. For each subject, we performed genotyping for nine single nucleotide polymorphisms in the MMP-7 gene to determine whether these polymorphisms might increase the expression of mucosal MMP-7 or increase the risk of developing gastric preneoplasia.
In our study population, the ratio of pepsinogens 1 and 2 (PG1/2 ratio) performed well as a diagnostic test for gastric mucosal preneoplasia. In combination with fasting serum gastrin concentration and H. pylori serology, the diagnostic accuracy was improved suggesting a role for these markers in clinical practice.
We have demonstrated that the gene transcript abundance of one mucosal protein – matrix metalloproteinase-7 (MMP-7) – was significantly increased in the presence of gastric preneoplasia. This effect was greater for disease phenotypes associated with higher cancer risk. These novel findings confirm the increased expression of MMP-7 in gastric epithilial preneoplasia and offer insight into areas of development for the future including the use of MMP-7 as a noninvasive biomarker of gastric preneoplasia. We also found that gastric preneoplasia was significantly more common in minor allele homozygotes for SNP rs17352054 than for major allele homozygotes and that carriage of the minor allele of SNP rs11225297 is associated with gastric preneoplasia amongst H. pylori seropositive individuals. We have also shown a significant influence on mucosal MMP-7 relative mRNA abundance of SNP genotypes for six of the polymorphisms examined.

Cell communication is central to the integration of cell function required for the development and homeostasis of multicellular animals. Proteins are an important currency of cell communication, acting locally (auto-, juxta-, or paracrine) or systemically (endocrine). The fibroblast growth factor (FGF) family contributes to the regulation of virtually all aspects of development and organogenesis, and after birth to tissue maintenance, as well as particular aspects of organism physiology. In the West, oncology has been the focus of translation of FGF research, whereas in China and to an extent Japan a major focus has been to use FGFs in repair and regeneration settings. These differences have their roots in research history and aims. The Chinese drive into biotechnology and the delivery of engineered clinical grade FGFs by a major Chinese research group were important enablers in this respect. The Chinese language clinical literature is not widely accessible. To put this into context, we provide the essential molecular and functional background to the FGF communication system covering FGF ligands, the heparan sulfate and Klotho co-receptors and FGF receptor (FGFR) tyrosine kinases. We then summarise a selection of clinical reports that demonstrate the efficacy of engineered recombinant FGF ligands in treating a wide range of conditions that require tissue repair/regeneration. Alongside, the functional reasons why application of exogenous FGF ligands does not lead to cancers are described. Together, this highlights that the FGF ligands represent a major opportunity for clinical translation that has been largely overlooked in the West.

The production of recombinant proteins such as the fibroblast growth factors (FGFs) is the key to establishing their function in cell communication. The production of recombinant FGFs in E. coli is limited, however, due to expression and solubility problems. HaloTag has been used as a fusion protein to introduce a genetically-encoded means for chemical conjugation of probes. We have expressed 11 FGF proteins with an N-terminal HaloTag, followed by a tobacco etch virus (TEV) protease cleavage site to allow release of the FGF protein. These were purified by heparin-affinity chromatography, and in some instances by further ion-exchange chromatography. It was found that HaloTag did not adversely affect the expression of FGF1 and FGF10, both of which expressed well as soluble proteins. The N-terminal HaloTag fusion was found to enhance the expression and yield of FGF2, FGF3 and FGF7. Moreover, whereas FGF6, FGF8, FGF16, FGF17, FGF20 and FGF22 were only expressed as insoluble proteins, their N-terminal HaloTag fusion counterparts (Halo-FGFs) were soluble, and could be successfully purified. However, cleavage of Halo-FGF6, -FGF8 and -FGF22 with TEV resulted in aggregation of the FGF protein. Measurement of phosphorylation of p42/44 mitogen-activated protein kinase and of cell growth demonstrated that the HaloTag fusion proteins were biologically active. Thus, HaloTag provides a means to enhance the expression of soluble recombinant proteins, in addition to providing a chemical genetics route for covalent tagging of proteins.

Biofilm structure and cell vitality in a laboratory model of subgingival plaque

The accumulation of dental plaque below the gingival margin (i.e. subgingival plaque) is responsible for the most prevalent microbe-induced diseases of humans--the periodontal diseases. Access to this plaque is difficult, making studies of its structure in vivo very difficult. We have, therefore, used a constant-depth film fermenter to grow microcosm subgingival dental plaques under conditions similar to those existing in vivo to enable us to study certain aspects of its structure. Confocal laser scanning microscopy revealed that the biofilms consisted of pillar-like stacks of bacteria separated by water channels. In terms of their structure, these microcosm dental plaques reached a pseudo steady-state after 4 days. Individual optical sections generally showed the outer layers of the biofilm stacks to contain a high proportion of nonviable cells surrounding an inner core of predominantly viable cells with "veins" of nonviable bacteria penetrating from the outer layers through into the inner core. Such a structure differs from that classically described for biofilms growing in an aerobic atmosphere where the arrangement of viable and nonviable bacteria is usually reversed. The reasons for the preponderance of nonviable bacteria in the outer layer of the bacterial stacks remain to be established.

Analysis of the effects of chlorhexidine on oral biofilm vitality and structure based on viability profiling and an indicator of membrane integrity

Multispecies biofilms modeling interproximal plaque were grown on a hydroxyapatite substratum in a constant-depth film fermentor and then immersed in a viewing solution containing fluorescent indicators of membrane integrity. Confocal laser scanning microscopy (CLSM) revealed the structure and spatial distribution of cell vitality within the biofilms. Chlorhexidine gluconate (CHX) was added to the viewing solution to achieve concentrations of 0.05 and 0.2% (wt/vol) before further CLSM time-lapse series were captured. Image analysis showed that exposure to 0.2% CHX caused the biofilm to contract at a rate of 1.176 micro m min(-1) along the z axis and also effected changes in total fluorescence measurements and viability profiles through the biofilms after a delay of 3 to 5 min. At a concentration of 0.05% CHX, total fluorescence measurements for the biofilm exhibited barely detectable changes after 5 min. Fluorescence profiles (fluorescence versus time versus depth), however, clearly showed that a time-dependent effect was present, but the clearest indicator of the effect of dilute CHX over time was viability profiling. These findings suggest the possibility of using fluorescent indicators of membrane integrity in conjunction with viability profiling to evaluate the penetration of the bactericidal effects of membrane-active antimicrobial compounds into biofilm.

Effects of dynamic fluid activity from an electric toothbrush on in vitro oral biofilms

<h4>Objectives</h4>To determine the plaque-removing ability of a Sonicare Plus electric toothbrush in an in vitro model.<h4>Material and methods</h4>Multispecies oral biofilms derived from human saliva were grown on hydroxyapatite discs in a constant-depth film fermenter. The biofilms were placed in a typodont model so that they mimicked the interproximal plaque between teeth 46 and 47 and were then treated with an electric toothbrush, both activated and inactivated. The distance from the bristle tips to the edge of the disc was 2.65 mm. Brushing action was controlled by a specially constructed brushing machine. After brushing, the number of viable bacteria removed from, and remaining in, the biofilms were determined.<h4>Results</h4>In all, 73.70% of viable bacteria in the biofilms were dislodged from the discs using the activated toothbrush. An inactivated toothbrush removed only 3.66%. Scanning electron microscopy and confocal microscopy revealed differences between untreated and treated biofilms.<h4>Conclusion</h4>The fluid shear forces generated by the electric toothbrush penetrated at least 2.65 mm beyond the reach of the bristles and these forces contributed to the toothbrush's plaque-removal ability (p<0.001).

Partner or Pariah? Building Social Capital with Clan Associations in Hong Kong’
This paper questions the economic perspective of institutions and its uncritical approach to using local organisations and leaders. It argues that the instrumental approach of socially-embedded institutions for enhancing project efficiency and political legitimacy disregards the existing unequal structures of authority within communities. Drawing from Bourdieu’s notion of ‘symbolic power’, I argue that local elites make use of collective rituals and cultural symbols to legitimatise their rule. Reinvention of traditions also places the poor at a disadvantage because they lack sufficient knowledge to draw upon ‘community wisdom’ to make claims and counter-claims. The romanticisation of socially-embedded institutions also plays down the dark side. A high level of trust can reduce transparency, making nepotism possible. The livelihoods of the poor, being built upon these exploitative relationships, also hinder them from challenging the inequalities.

Determining the strange quark mass for 2-flavour QCD

Using the O(a) Symanzik improved action an estimate is given for the strange
quark mass for unquenched (n_f=2) QCD. The determination is via the axial Ward
identity (AWI) and includes a non-perturbative evaluation of the
renormalisation constant. Numerical results have been obtained at several
lattice spacings, enabling the continuum limit to be taken. Our results
indicate a value for the strange quark mass (in the MS-bar scheme at a scale of
2 GeV) in the range 100 - 130 MeV. A comparison is also made with other recent
lattice determinations of the strange quark mass using dynamical sea quarks.

Perturbative Renormalisation for Low Moments of Generalised Parton

We present the non-forward quark matrix elements of operators with one and
two covariant derivatives needed for the renormalisation of the first and
second moments of generalised parton distributions in one-loop lattice
perturbation theory using clover fermions. For some representations of the
hypercubic group commonly used in simulations we define the sets of possible
mixing operators and compute the one-loop mixing matrices of renormalisation
factors. Tadpole improvement is applied to the results and some numerical
examples are presented.

Geometric transitions, flops and non-Kähler manifolds: II

We continue our study of geometric transitions in type II and heterotic theories. In type IIB theory we discuss an F-theory setup which clarifies many of our earlier assumptions and allows us to study gravity duals of N = 1 gauge theories with arbitrary global symmetry group G. We also point out the subtle differences between global and local metrics, and show that in many cases the global descriptions are far more complicated than discussed earlier. We determine the full global description in type I/heterotic theory. In type IIA, our analysis gives rise to a local non-Kahler metric whose global description involves a particular orientifold action with gauge fluxes localised on branes. We are also able to identify the three form fields that allow for a smooth flop in the M-theory lift. We briefly discuss the issues of generalised complex structures in type IIB theory and possible half-twisted models in the heterotic duals of our type II models. In a companion paper we will present details on the topological aspects of these models.

Two loop correction to the Gribov mass gap equation in Landau gauge QCD

We determine the two loop correction to Gribov's mass gap equation for
quantum chromodynamics in the Landau gauge in the MSbar scheme by computing the
two loop correction to the horizon condition derived from Zwanziger's local
renormalizable Lagrangian which incorporates the Gribov parameter. We verify
that with the explicit result, the two loop ghost propagator is enchanced in
the infrared.

B_s meson excited states from the lattice

This is a follow-up to our earlier work [Phys. Rev. D 65, 014512 (2002); Eur.
Phys. J. C 28, 79 (2003); Phys. Rev. D 69, 094505 (2004)] for the energies and
the charge (vector) and matter (scalar) distributions for S-wave states in a
heavy-light meson, where the heavy quark is static and the light quark has a
mass about that of the strange quark. We study the radial distributions of
higher angular momentum states, namely P- and D-wave states. In nature the
closest equivalent of this heavy-light system is the B_s meson.
The calculation is carried out with dynamical fermions on a 16^3 times 32
lattice with a lattice spacing of about 0.10 fm generated with the
non-perturbatively improved clover action. It is shown that several features of
the energies and radial distributions are in qualitative agreement with what
one expects from a simple one-body Dirac equation interpretation.

Matrix elements and baryon spectroscopy from unquenched lattice QCD with
improved staggered quarks

We look for the effect of open decay channels on the masses of baryons in
unquenched lattice QCD. We apply variational smearing using fuzzed basis states
to a staggered nucleon operator. The signal for the s_bar s current in the
nucleon is studied. The lattice calculations are done using improved staggered
fermions.

Pseudoscalar singlet physics with staggered fermions

We report on progress in measuring disconnected correlators associated with pseudoscalar flavor-singlet mesons. This will eventually allow us to compute the masses of the eta and eta' mesons. Flavor-singlet physics also presents an interesting test of the staggered fermion formulation, as disconnected correlators are sensitive to whether the same action governs both sea quarks and valence quarks. It can also help test the validity of the ``fourth-root trick'' used in unquenched lattice calculations where the number of flavors $N_f

Scalar glueball and meson spectroscopy in unquenched lattice QCD with improved staggered quarks

We present results of an exploratory study of singlet scalar states in unquenched QCD using both glueball and meson operators. Results for non-singlet non-strange scalar mesons are also presented. We use Asqtad improved staggered fermions and gauge configurations generated by the MILC collaboration at lattice spacings of .12 and .09 fm. In this formulation, the glueball mass is not significantly different from the quenched value at finite lattice spacing. Significant taste violations are present in the scalar sector. At light quark masses, decay channels complicate the mass determinations. There is some evidence that the non-strange singlet meson lies below the non-singlet meson.

A determination of the strange quark mass for unquenched clover fermions
using the AWI

Using the O(a) Symanzik improved action an estimate is given for the strange
quark mass for unquenched (nf=2) QCD. The determination is via the axial Ward
identity (AWI) and includes a non-perturbative evaluation of the
renormalisation constant. Numerical results have been obtained at several
lattice spacings, enabling the continuum limit to be taken. Results indicate a
value for the strange quark mass (in the MSbar-scheme at a scale of 2GeV) in
the range 100 - 130MeV.

Estimating the Unquenched Strange Quark Mass from the Lattice Axial Ward

We present a determination of the strange quark mass for two flavours (nf=2)
of light dynamical quarks using the axial Ward identity. The calculations are
performed on the lattice using O(a) improved Wilson fermions and include a
fully non-perturbative determination of the renormalisation constant. In the
continuum limit we find in the MSbar scheme at 2GeV, ms = 111(6)(4)(6)MeV using
the force scale r0 = 0.467fm, where the first error is statistical, the second
and third are systematic due to the fit and scale uncertainties respectively.
Results are also presented for the light quark mass and the chiral condensate.
The corresponding results are also given for r0=0.5fm.

We present recent results for meson decay constants calculated on
configurations with two flavours of O(a)-improved Wilson fermions.
Non-perturbative renormalisation is applied and quark mass dependencies as well
as finite volume and discretisation effects are investigated. In this work we
also present a computation of the coupling of the light vector mesons to the
tensor current using dynamical fermions.

Non-perturbative renormalisation for overlap fermions

Using non-perturbative techniques we have found the renormalisation factor,
Z, in the RI-MOM scheme for quark bilinear operators in quenched QCD. We worked
with overlap fermions using the Luescher-Weisz gauge action. Our calculation
was performed at beta=8.45 at a lattice spacing of 1/a=2.1 GeV using a value of
rho=1.4. Our results show good agreement between the vector and the axial
vector in the zero mass limit. This shows that overlap fermions have good
chiral properties. To attempt to improve the discretisation errors in our
results we subtracted the O(a^2) terms in one-loop lattice perturbation theory
from the Monte Carlo Green functions. In particular we paid attention to the
operators for the observable <x>. We found a value for the renormalisation
constants Z^msbar_(v_2b) and Z^msbar_(v_2a) just less than 1.9 at mu=1/a=2.1
GeV.

Stochastic perturbation theory and the gluon condensate

On the lattice searching for the gluon condensate is difficult because a large perturbative contribution to the expectation value of the action has to be subtracted before looking for a small contribution from a possible gluon condensate. The perturbative calculation therefore has to be very precise. We use a modified version of stochastic perturbation theory to calculate a perturbative series in a boosted coupling, which converges more rapidly than the series with the usual lattice coupling, reducing the uncertainties in our results. We do not see any condensate of dimension two, as suggested by some earlier lattice studies, but we do find a contribution from a dimension four condensate. The value of this condensate is approximately 0.04(1) GeV^4, but with large uncertainties.

Perturbative renormalisation of quark bilinear operators for overlap
fermions with and without stout links and improved gauge action

We calculate lattice renormalisation constants of local and one-link quark
operators for overlap fermions and improved gauge actions in one-loop
perturbation theory. For the local operators we stout smear the SU(3) links in
the fermionic action. Using the popular tadpole improved L\"uscher-Weisz
actions at $\beta=8.45$ and $\beta=8.0$ we present numerical values for the Z
factors in the $\bar{MS}$ scheme (partly as function of the stout smearing
strength). We compare various levels of mean field (tadpole) improvement which
have been applied to our results.

Twisted mass fermions allow light quarks to be explored but with the
consequence that there are mass splittings, such as between the neutral and
charged pion. Using a direct calculation of the connected neutral pion
correlator and stochastic methods to evaluate the disconnected correlations, we
determine the neutral pion mass. We explore the dependence on lattice spacing
and quark mass in quenched QCD. For dynamical QCD, we determine the sign of the
splitting which is linked, via chiral PT, to the nature of the phase transition
at small quark mass.

We present recent Monte Carlo data for the axial charge of the nucleon
obtained by the QCDSF-UKQCD collaboration for N_f=2 dynamical quarks. We
compare them with formulae from chiral perturbation theory in finite and
infinite volume and find a remarkably consistent picture.

Hadronic decays
Michael, C
Hadronic decays and transitions are a key ingredient of hadronic physics. I discuss how hadronic decays can be explored in lattice gauge theory and review studies undertaken. I also discuss the impact of decays on masses and how lattice studies can explore the nature of a hadronic state: namely whether it is a molecular or quark-antiquark state. A brief discussion of lattice exploration of pentaquark states is presented.

The effect of sea quarks on the mass of the charm quark from Lattice QCD

We compute the mass of the charm quark using both quenched and dynamical
lattice QCD calculations. We examine the effects of mass dependent lattice
artifacts by comparing two different formalisms for the heavy quarks. We take
the continuum limit of the charm mass in quenched QCD by extrapolating from
three different lattice spacings. At a fixed lattice spacing, the mass of the
charm quark is compared between quenched QCD and dynamical QCD with a sea quark
mass around strange. In the continuum limit of quenched QCD, we find
m_c(m_c)=1.29(7)(13) GeV. No evidence was seen for unquenching.

The ghost condensate εabcc̄bcc is considered together with the gluon condensate Aμ2 in SU(2) Euclidean Yang-Mills theories quantized in the Landau gauge. The vacuum polarization ceases to be transverse due to the nonvanishing condensate εabcc̄bcc. The gluon propagator itself remains transverse. By polarization effects, this ghost condensate induces then a splitting in the gluon mass parameter, which is dynamically generated through Aμ2. The obtained effective masses are real when Aμ2 is included in the analysis. In the absence of Aμ2, the already known result that the ghost condensate induces effective tachyonic masses is recovered. At the one-loop level, we find that the effective diagonal mass becomes smaller than the off-diagonal one. This might serve as an indication for some kind of Abelian dominance in the Landau gauge, similar to what happens in the maximal Abelian gauge. © 2006 The American Physical Society.

The nonlocal mass operator Trd4xFμν(D2)-1Fμν is considered in Yang-Mills theories in Euclidean space-time. It is shown that the operator Trd4xFμν(D2)-1Fμν can be cast in local form through the introduction of a set of additional fields. A local and polynomial action is thus identified. Its multiplicative renormalizability is proven by means of the algebraic renormalization in the class of linear covariant gauges. The anomalous dimensions of the fields and of the mass operator are computed at one-loop order. A few remarks on the possible role of this operator for the issue of the gauge invariance of the dimension two condensates are outlined. © 2005 The American Physical Society.

We study the flavour breaking effects appearing in the Wilson twisted mass
formulation of lattice QCD. In this quenched study, we focus on the mass
splitting between the neutral and the charged pion, determining the neutral
pion mass with a stochastic noise method to evaluate the disconnected
contributions. We find that these disconnected contributions are significant.
Using the Osterwalder-Seiler interpretation of the connected piece of the
neutral pion correlator, we compute the corresponding neutral pion mass to
study with more precision the scaling behaviour of the mass splitting.

Higgs--Matter splitting in quasi-realistic orbifold string GUTs

E6 grand unification combines the Standard Model matter and Higgs states in the single 27 representation. I discuss how the E6 structure underlies the quasi-realistic free fermion heterotic-string models. E6 -> SO(10) X U(1) breaking is obtained by a GSO phase in the N=1 partition function. The equivalence of this symmetry breaking phase with a particular choice of a boundary condition basis vector, which is used in the quasi-realistic models, is demonstrated in several cases. As a result matter states in the spinorial 16 representation of SO(10) arise from the twisted sectors, whereas the Higgs states arise from the untwisted sector. Possible additional phenomenological implications of this E6 symmetry breaking pattern are discussed.

We consider the sparticle spectra that arise when anomaly mediation is the source of supersymmetry-breaking and the tachyonic slepton problem is solved by a Fayet-Iliopoulos D-term. We also show how this can lead to a minimal viable extension of anomaly mediation, in which the gauge symmetry associated with this D-term is broken at very high energies, leaving as its footprint in the low energy theory only the required D-terms and seesaw neutrino masses. © 2005 Elsevier B.V. An estimate of the chiral condensate from unquenched lattice QCD
McNeile, Craig
Using the parameters in the chiral Lagrangian obtained by MILC from their
unquenched lattice QCD calculations with 2+1 flavours of sea quarks, I estimate
<psi_bar psi> (2 GeV)/n_f = -(259 \pm 27 MeV)**3 in the MS_bar scheme. I
compare this value to other determinations.

Three loop MSbar renormalization of QCD in the maximal abelian gauge

We determine the three loop anomalous dimensions of the quark, centre and
off-diagonal gluons, centre and off-diagonal ghosts and the gauge fixing
parameters in the maximal abelian gauge for an arbitrary colour group in the
MSbar renormalization scheme at three loops. We show that the three loop MSbar
beta-function emerges from the renormalization of the centre gluon and also
deduce the anomalous dimension of the BRST invariant dimension two mass
operator. Moreover, we demonstrate that in the limit that the dimension of the
centre of the group tends to zero, the anomalous dimensions of the quarks,
off-diagonal gluons and off-diagonal ghosts tend to those of the quarks, gluons
and ghosts of the Curci-Ferrari gauge respectively.

We compute lattice renormalisation constants of one-link quark operators
({\it i.e.} operators with one covariant derivative) for overlap fermions and
L\"uscher-Weisz gauge action in one-loop perturbation theory. Among others,
such operators enter the calculation of moments of polarised and unpolarised
hadron structure functions. Results are given for \beta=8.45, \beta=8.0 and
mass parameter \rho=1.4, which are commonly used in numerical simulations. We
apply mean field (tadpole) improvement to our results.

A Determination of the Lambda Parameter from Full Lattice QCD

We present a determination of the QCD parameter Lambda in the quenched
approximation (n_f=0) and for two flavours (n_f=2) of light dynamical quarks.
The calculations are performed on the lattice using O(a) improved Wilson
fermions and include taking the continuum limit. We find Lambda_{n_f=0} =
259(1)(20) MeV and Lambda_{n_f=2} = 261(17)(26) MeV}, using r_0 = 0.467 fm to
set the scale. Extrapolating our results to five flavours, we obtain for the
running coupling constant at the mass of the Z boson alpha_s(m_Z) =
0.112(1)(2). All numbers refer to the MSbar scheme.

Moduli fixing in realistic string vacua

I illustrate the existence of quasi-realistic heterotic-string models in
which all the untwisted Kaehler and complex structure moduli, as well as all of
the twisted sectors moduli, are projected out by the generalized GSO
projections. I discuss the conditions and characteristics of the models that
produce this result. The existence of such models offers a novel perspective on
the realization of extra dimensions in string theory. In this view, while the
geometrical picture provides a useful mean to classify string vacua, in the
phenomenologically viable cases there is no physical realization of extra
dimensions. The models under consideration correspond to Z2 X Z2 orbifolds of
six dimensional tori, plus additional identifications by internal shifts and
twists. The special property of the Z2 X Z2 orbifold is that it may act on the
compactified dimensions as real, rather than complex, dimensions. This property
enables an asymmetric projection on all six internal coordinates, which enables
the projection of all the untwisted Kaehler and complex structure moduli.

Blind and semi-blind equalisation for Multiple-Input Multiple-Output wireless communication systems using Independent Component Analysis

This thesis proposes novel blind and semi-blind receivers for Multiple-Input Multiple-Output (MIMO) wireless communication systems employing Orthogonal Frequency Division Multiplexing (OFDM) or Single-Carrier Frequency Domain Equalisation (SCFDE) modulation. MIMO systems can achieve higher data rates than Single-Input Single-Output (SISO) systems while the use of OFDM and SC-FDE is motivated by their simple equalisation of frequency selective channels. Furthermore, the blind or semi-blind operation increases the bandwidth efciency compared to traditional training based receivers, which is particularly interesting in MIMO systems since they require more training data than SISO systems. Blind and semi-blind equalisation in this work is performed using the statistical tool of Independent Component Analysis (ICA), which relies on the independence of the source streams. First, novel blind receivers for MIMO OFDM systems are proposed. Two linear receivers, which have different complexity / performance tradeoffs are presented. The indeterminacies of ICA estimates are handled by post-processing of the estimates, which relies on precoding of the source streams. Subsequently, these receivers are combined with Layered Space-Frequency Equalisation (LSFE), which results in performance close to the case with perfect Channel State Information (CSI) at the receiver. Reduced-complexity versions are also considered, which are essential for OFDM systems employing a large number of subcarriers. Furthermore, performance and computational complexity comparisons with other published blind receivers show the superiority of the proposed methods. Next, semi-blind receivers for MIMO OFDM systems are proposed, which are derived from the blind methods above. The advantage of operating semi-blindly is that the indeterminacies of the ICA estimates are avoided. Furthermore, better performance than in the blind case is obtained at low to moderate Signal to Noise Ratios (SNRs) and Doppler frequency shifts, using a training overhead of 2%. However, for high SNRs or Doppler frequency shifts as well as with reduced-complexity versions convergence to a solution extracting the sources in the wrong order can occur, which results in an error floor, since no post-processing to handle these ICA indeterminacies is used. Finally, semi-blind receivers for MIMO SC-FDE systems using linear equalisation and LSFE are investigated. To the author's knowledge this is the first published application of ICA to MIMO SC-FDE systems. Since in SC-FDE systems the frequency domain signals are Gaussian, ICA is applied in time domain while a computationally efficent frequency domain equalisation is employed. The resulting receiver is shown to achieve performance close to the perfect CSI case for moderate to high SNRs, using a training overhead of only 0:05%. Furthermore, convergence to a wrong solution as in the semi-blind OFDM case has not been observed, which can be explained by the larger solution space in SC-FDE systems than in OFDM systems.

Searching for chiral logs in the static-light decay constant

Using the clover fermion action in unquenched QCD with pion masses as low as
420 MeV, we look for evidence for chiral logs in the static-light decay
constant. There is some evidence for a chiral log term, if the original static
theory of Eichten and Hill is used. However, the more precise data from the
static action of the ALPHA collaboration do not show any evidence for
non-linear dependence of the static-light decay constant on the light quark
mass. We make some comments on the connection between chiral perturbation
theory for decay constants of the pion and static-light meson.

P-wave Radial distributions of a Heavy-light meson on a lattice

This is a follow-up to our earlier work for the charge (vector) and matter (scalar) distributions for S-wave states in a heavy-light meson, where the heavy quark is static and the light quark has a mass about that of the strange quark. The calculation is again carried out with dynamical fermions on a 16^3x24 lattice with a lattice spacing of about 0.14 fm. It is shown that several features of the S- and P-wave distributions are in qualitative agreement with what one expects from a simple one-body Dirac equation interpretation.

low moments of the unpolarised nucleon structure functions is given. Particular

Perturbative Renormalisation of the Second Moment of Generalised Parton

We calculate the non-forward quark matrix elements of operators with two
covariant derivatives needed for the renormalisation of the second moment of
generalised parton distributions in one-loop lattice perturbation theory using
Wilson fermions. For some representations of the hypercubic group commonly used
in simulations we determine the sets of all possible mixing operators. For
those representations the one-loop mixing matrices of renormalisation factors
are found. Due to non-vanishing contributions of operators with external
ordinary derivatives the number of contributing operators increases compared to
forward matrix elements.

Bleeding after dental extractions in patients taking warfarin.

<h4>Objective</h4>To assess the incidence of bleeding after dental extractions in subjects taking warfarin continuously before and after extractions whose International Normalised Ratio (INR) was below 4.0 at the time of extraction.<h4>Methods</h4>This was a case series study of 150 patients without controls who required extraction of at least one tooth under local anaesthetic. All sockets were subsequently packed with absorbable oxycellulose and sutured.<h4>Results</h4>A total of 58 women and 92 men were included (mean age 66 years); their ages were similar. The mean INR (S.D.) was 2.5 (0.56), although most patients had an INR less than 2.5 (n=101). Ten patients (7%) bled after extraction, enough to require a return to hospital. Five patients of 101 with an INR</=2.5, and 5 with an INR>2.5 out of 49 bled after extraction (p=0.29). Bleeding after extraction was not associated with operative antibiotics. All patients who bled were managed conservatively and none was admitted to hospital.<h4>Conclusion</h4>Patients taking warfarin whose INR is up to 4.0 and who have dental extractions in hospital do not have clinically significant bleeds post-operatively.

IntroductionThe use of Quantitative Trait Loci (QTL) data is increasingly used to aid in the discovery of candidate genes involved in phenotypic variation. Tens to hundreds of genes, however, may lie within even well defined QTL. It is therefore vital that the identification, selection and functional testing of candidate Quantitative Trait genes (QTg) are carried out systematically, and without bias [1]. With the advent of microarrays, researchers are able to directly examine the expression of all genes on a genome wide scale, including those underlying QTL regions.The scale of data being generated by such high-throughput experiments has led some investigators to follow a hypothesis-driven approach [2]. Although these techniques for candidate gene identification are valid, they run the risk of overlooking genes that have less obvious associations with the phenotype. By making selections based on prior assumptions of what processes may be involved, the genes that may actually be involved in the phenotype can be overlooked. A further complication is that the use of ad hoc methods for candidate gene identification are inherently difficult to replicate and are compounded by poor documentation of the methods used to generate and capture the data from such investigations in published literature.With an ever increasing number of institutes offering programmatic access to their resources in the form of web services, however, experiments previously conducted manually can now be replaced by automated experiments, capable of processing a far greater volume of data. By reconstructing the original investigation methods in the form of workflows, we are now able to pass data directly from one service to the next. This enables us to process the data in a much more systematic, un-biased, and explicit manner.MethodsWe propose a data-driven methodology that identifies the known pathways that intersect a QTL and those derived from a set of differentially expressed genes from a microarray study. This methodology is implemented systematically through the use of web services and workflows. For the purpose of implementing this systematic pathway-driven approach, we have chosen to use the Taverna workbench [3].Results and DiscussionPreliminary studies into the modes of resistance to African Trypanosomiasis were carried out for the mouse model organism. These studies illustrated how the large-scale analysis of microarray gene expression and QTL data, investigated at the level of biological pathways, enables links between genotype and phenotype to be successfully established [4]. This approach was implemented systematically through the use of explicitly defined workflows.

Unravelling referral paths relating to the dental care of children: a study in Liverpool.

<h4>Objective</h4>To describe primary care referral networks relating to children's dental care and the main influences on referral decisions taken by dentists working in a primary care setting.<h4>Design</h4>A postal questionnaire to all 130 general dental practitioners (GDPs) in contract with Primary Care Trusts (PCTs), and 24 Community Dental Service (CDS) dentists in Liverpool.<h4>Outcome measures</h4>Characteristics of patient groups and factors influencing the choice of referral pathway of children referred from primary dental care.<h4>Results</h4>There were good responses rates (110 [85%] GDPs and 22 [92%] CDS dentists). The two main reasons why GDPs referred children to hospitals were (a) for treatment under general anaesthetic (GA) or relative analgesia (RA) and (b) for restorative care of dentally anxious children. GDPs also referred anxious children requiring simple restorative care and/or RA to the CDS. Only eight GDPs (7%) cited a lack of experience as a reason for referral of dentally anxious children for simple restorative care, compared to 53 (48%) who cited a lack of RA facilities, and 25 (23%) who cited financial considerations.<h4>Conclusions</h4>GDPs refer children to both hospital services and the CDS, and identify a lack of RA facilities and economic pressures as key reasons for referral.

Policy implications of a psychological model of mental disorder

Background: Mental health care in the United Kingdom is rapidly changing, but many commentators, particularly sociologists and psychologists, view systems as remaining wedded to a medical model and not "fit for purpose". If services are to improve, the way that mental health problems are understood by the services providing care needs to change radically. Aims: To illustrate how a psychological model of mental disorder-the "mediating psychological processes model"-could assist mental health service policy development and implementation. Method: A review of selected literature and policy documents was conducted. Results: A "manifesto" for mental health service policy is presented, based on psychological principles. Conclusions: Psychological models, at least in addition to medical approaches, could better inform policy and service design. © Shadowfax Publishing and Informa UK Ltd.

Kinderman (2005) presented a psychological model of mental disorder, based on a critique and reformulation of the biopsychosocial model. Kinderman suggested that disruption or dysfunction in psychological processes is a final common pathway in the development of mental disorder. These processes include, but are not limited to, cognitive processes. This 'mediating psychological processes model' proposes that biological and environmental factors, together with a person's personal experiences, lead to mental disorder through their conjoint effects on these psychological processes. The clinical implications of this model are discussed further here. It is proposed that formulations rather than diagnoses should predominate clinical planning, that these formulations should detail the hypothesised disruption to psychological processes or mechanisms, that psychological therapies should receive higher priority, and that medical, social and even psychological interventions are most likely to be clinically effective if they are designed on the basis of their likely beneficial impact on underlying psychological mechanisms. © 2006 British Association for Behavioural and Cognitive Psychotherapies.

Illness beliefs in schizophrenia

Beliefs about health and illness shape emotional responses to illness, health-related behaviour and relationships with health-care providers in physical illness. Researchers are beginning to study the illness beliefs of people with psychosis, primarily using models developed in relation to physical illness. It is likely that modifications to these models will be necessary if they are to apply to mental disorders, and it is probable that some of the assumptions underlying the models will be inappropriate. In particular, different dimensions of understanding may be present in mental illness in comparison to those identified in physical illness. The present study examines the beliefs of 20 patients in the UK diagnosed with schizophrenia, including 10 currently psychotic inpatients and 10 outpatients in remission, about their experiences, using qualitative interviews and thematic analysis. Patients currently experiencing psychosis did not identify their experiences as separable 'illnesses' and did not have 'illness beliefs'. Patients currently in a period of remission appraised their experiences as distinct from their own normal behaviour, but used conceptual frameworks of understanding that deviated significantly from conventional 'health belief' models. Patients' ways of understanding mental illness did not parallel those described in physical illnesses. Methods for assessing beliefs about mental illness should therefore not be transferred directly from studies of beliefs about physical illness, but should be tailored to the nature of patients' beliefs about mental illness.

An interacting cognitive subsystems approach to personality disorder

<jats:title>Abstract</jats:title><jats:p>Personality disorder is defined as an enduring pattern of problems with cognition, emotional regulation and behaviour. Personality disorder is a common problem, but also a serious challenge for mental health services. A theoretical psychological model of personality disorder that is based on a scientific understanding of the architecture of cognition would provide a logical basis for treatment and further study. This paper proposes that the interacting cognitive subsystems model of a distributed architecture for human cognition can provide a theoretical account of personality disorder. Two inherent aspects of the ICS model, ‘non‐discriminative processing’ and ‘interlock’, are discussed. Non‐discriminative processing occurs when novel material is recognized as discrepant with existing expectations, but does not lead to a modification of existing representational schemata. Interlock refers to a situation where the unchanged output of subsystem processing is used as the input for another subsystem in a feedback loop. It is hypothesized that these two processes, together with abnormal content of the implicational representational schema, can explain the rigidity and maladaptive nature of cognitive schemata that are themselves believed to lead to the clinical features of personality disorder. EVOLVING FORMULATIONS: SHARING COMPLEX INFORMATION WITH CLIENTS

<jats:p>Psychological formulations are central to cognitive behavioural approaches. The use of such formulations presents a number of difficulties when working with clients with psychotic problems. Despite this, sophisticated psychological formulations can be collaboratively developed with psychotic clients. This paper presents one method of developing such formulations through an evolutionary process. Early in the therapeutic process, simple formulations involving straightforward theoretical models are presented, which are systematically elaborated as therapy proceeds. This involves developing, collaboratively with clients, successive layers of formulation. Each of these layers builds on and incorporates the previous one, yet involves an incremental increase in complexity, depth and informational content. The evolutionary process is illustrated with a case example.</jats:p>

The efficacy of acupuncture in the treatment of temporomandibular joint myofascial pain: a randomised controlled trial.

<h4>Objectives</h4>To compare the effect of real acupuncture and sham acupuncture in the treatment of temporomandibulat joint myofascial pain, in order to establish the true efficacy of acupuncture.<h4>Methods</h4>A double blind randomised controlled trial conducted in the TMD Clinic, at the School of Dentistry, The University of Manchester. Twenty-seven patients were assigned to one of two treatment groups. Group 1 received real acupuncture treatment whilst Group 2 received a sham acupuncture intervention. Both the assessor and the patient were blinded regarding the group allocation. Baseline assessment of the outcome variables was made prior to the first treatment session, and was repeated following the last treatment.<h4>Results</h4>The results demonstrated that real acupuncture had a greater influence on clinical outcome measure of TMJ MP than those of sham acupuncture, and the majority of these reached a level of statistical significance.<h4>Conclusion</h4>Acupuncture had a positive influence on the signs and symptoms of TMJ MP. In addition, this study provides evidence that the Park Sham Device was a credible acupuncture control method for trials involving facial acupoints.

Quality-of-life trajectories in epilepsy: A review of the literature

The potential psychosocial sequelae of epilepsy are well-documented, but it cannot be assumed that trajectories for quality of life (QOL) of people with epilepsy will inevitably follow its clinical course. In this article, we draw on available literature to suggest likely QOL trajectories associated with epilepsy and the broad range of disease-, patient-, and other-focused factors that appear important in determining them. We conclude that both the likely shape and time frame for QOL trajectories associated with particular clinical scenarios can be delineated, but that their shape can be altered by a much wider range of factors than those represented as epilepsy disease progression. We identify contributory factors currently relatively unexplored and highlight implications for treatment and areas for future research.

Phenomenology of static-light mesons from unquenched lattice QCD

We present results for the static-light meson from unquenched lattice QCD.
The unquenched gauge configurations were generated using the non-perturbatively
improved clover action. At the fixed lattice spacing of 0.1 fm the lightest sea
quark mass used is a third of the strange quark mass. A comparison is made
between heavy-light chiral perturbation theory and the f_B^(static) decay
constant. The mass of the bottom quark is also reported: m_b(m_b) = 4.25(2)(11)
GeV, where the first error is statistical and the last error is the systematic
uncertainty.

Determination of Light and Strange Quark Masses from Full Lattice QCD

We compute the light and strange quark masses m_l = (m_u+m_d)/2 and m_s,
respectively, in full lattice QCD with N_f=2 flavors of light dynamical quarks.
The renormalization constants, which convert bare quark masses into
renormalized quark masses, are computed nonperturbatively, including the effect
of quark-line disconnected diagrams. We obtain m_l=4.7(2)(3) MeV and
m_s=119(5)(8) MeV in the MSbar scheme at the scale 2 GeV.

One-loop renormalisation for the second moment of GPDs with Wilson

We calculate the non-forward quark matrix elements for operators with two
covariant derivatives in one-loop lattice perturbation theory using Wilson
fermions. These matrix elements are needed in the renormalisation of the second
moment of generalised parton distributions measured in lattice QCD. For some
commonly used representations of the hypercubic group we determine the sets of
all mixing operators and find the matrices of mixing and renormalisation
factors.

Determination of Lambda in quenched and full QCD - an update

We present an update on our previous determination of the Lambda parameter in
QCD. The main emphasis is on results for two flavours of light dynamical
quarks, where we can now almost double the amount of data used, including
values at smaller lattice spacings. The calculations are performed using $O(a)$
improved Wilson fermions. Little change is found to previous numerical values.

Massive renormalizable Yang-Mills theories in three dimensions are analyzed within the algebraic renormalization in the Landau gauge. In analogy with the four-dimensional case, the renormalization of the mass operator AμaAμa turns out to be expressed in terms of the fields and coupling constant renormalization factors. We verify the relation we obtain for the operator anomalous dimension by explicit calculations in the large Nf expansion. The generalization to other gauges such as the non-linear Curci-Ferrari gauge is briefly outlined. © 2004 Elsevier Inc. All rights reserved.

The Malawi National Tuberculosis Programme: an equity analysis.

<h4>Background</h4>Until 2005, the Malawi National Tuberculosis Control Programme had been implemented as a vertical programme. Working within the Sector Wide Approach (SWAp) provides a new environment and new opportunities for monitoring the equity performance of the programme. This paper synthesizes what is known on equity and TB in Malawi and highlights areas for further action and advocacy.<h4>Methods</h4>A synthesis of a wide range of published and unpublished reports and studies using a variety of methodological approaches was undertaken and complemented by additional analysis of routine data on access to TB services. The analysis and recommendations were developed, through consultation with key stakeholders in Malawi and a review of the international literature.<h4>Results</h4>The lack of a prevalence survey severely limits the epidemiological knowledge base on TB and vulnerability. TB cases have increased rapidly from 5,334 in 1985 to 28,000 in 2006. This increase has been attributed to HIV/AIDS; 77% of TB patients are HIV positive. The age/gender breakdown of TB notification cases mirrors the HIV epidemic with higher rates amongst younger women and older men. The WHO estimates that only 48% of TB cases are detected in Malawi. The complexity of TB diagnosis requires repeated visits, long queues, and delays in sending results. This reduces poor women and men's ability to access and adhere to services. The costs of seeking TB care are high for poor women and men - up to 240% of monthly income as compared to 126% of monthly income for the non-poor. The TB Control Programme has attempted to increase access to TB services for vulnerable groups through community outreach activities, decentralising DOT and linking with HIV services.<h4>Conclusion</h4>The Programme of Work which is being delivered through the SWAp is a good opportunity to enhance equity and pro-poor health services. The major challenge is to increase case detection, especially amongst the poor, where we assume most 'missing cases' are to be found. In addition, the Programme needs a prevalence survey which will enable thorough equity monitoring and the development of responsive interventions to promote service access amongst 'missing' women, men, boys and girls.

An unquenched lattice QCD calculation of the mass of the bottom quark

We analyse the effect of three-loop running on the sparticle spectrum for the MSSM Snowmass benchmark points. <h4>Introduction</h4>China has an estimated 5 million people with tuberculosis (TB). Official policy is that treatment of all patients is directly observed by health workers; completion rates are reported to be in excess of 90%, and drugs should be supplied for free. However, some research suggests there is a gap between the official policies and practice.<h4>Methods</h4>Survey of TB patients in four counties of one municipality; record assessment at one TB centre; patient and village doctor in-depth interviews.<h4>Results</h4>Sixteen per cent (64/401) reported being directly observed every time they took treatment; less than 5% of TB patients (17/401) were observed by health staff. Overall, 12.5% (50/401) reported they had not taken any TB drugs in the previous week, but this varied between the four counties (range 6.2 to 21.7%). We used survival analysis with medical records at one centre: 74.1% of new patients collected their drugs for their sixth month of treatment, and 50.3% attended the final visit at 6 months. Qualitative research indicated direct observation is neither well understood nor thought to be necessary, and that patients reported being charged expensive fees for ancillary treatments, such as liver protection drugs.<h4>Conclusion</h4>In China, direct observation is not well implemented and may not be a feasible policy option. Official completion rates are higher than we found in this study. The concept of free treatment has become blurred, with charges for additional tests and drugs, especially liver protection drugs. The government is already actively tackling these issues, and involvement of managers and others in this process will be helpful.

Most research into the early telephone system has focused on telephone providers rather than users, and this article begins to address that imbalance. The telephone was initially used to improve internal communications within firms, by connecting offices with warehouses, or by enabling staff working away from the office to report back. With the expansion of exchange networks, the commercial, intermediary and brokering sectors became heavy users of the technology for routine information transfer within business districts. Business elites continued to favour face-to-face contact for strategic business negotiations, however, and delegated telephone use to their employees.

One loop MS gluon pole mass from the local composite operator formalism

We compute the one loop corrections to the pole mass of the gluon in the
MSbar scheme in the Landau gauge in both the Curci-Ferrari model and the local
composite operator formalism with Nf flavours of massless quarks. For the
latter we determine an estimate for the gluon mass using the effective
potential of a local dimension two composite operator and find, for example,
m_{gluon} = 2.10 Lambda_MSbar in Yang-Mills theory.

Analytic study of the off-diagonal mass generation for Yang-Mills theories in the maximal Abelian gauge

We investigate a dynamical mass generation mechanism for the off-diagonal gluons and ghosts in SU(N) Yang-Mills theories, quantized in the maximal Abelian gauge. Such a mass can be seen as evidence for the Abelian dominance in that gauge. It originates from the condensation of a mixed gluon-ghost operator of mass dimension two, which lowers the vacuum energy. We construct an effective potential for this operator by a combined use of the local composite operators technique with the algebraic renormalization and we discuss the gauge parameter independence of the results. We also show that it is possible to connect the vacuum energy, due to the mass dimension-two condensate discussed here, with the nontrivial vacuum energy originating from the condensate.〈A μ2〉, which has attracted much attention in the Landau gauge.

Hadronic decay of a scalar B meson from the lattice

We explore the transitions B$(0^+)$ to B $\pi$ and B$_s(0^+)$ to B K from
lattice QCD with $N_f=2$ flavours of sea quark, using the static approximation
for the heavy quark. We evaluate the effective coupling constants, predicting a
B$(0^+)$ to B $\pi$ width of around 160 MeV. Our result for the coupling
strength adds to the evidence that the B$_s(0^+)$ meson is not predominantly a
molecular state (BK).

Improved Wilson QCD simulations with light quark masses

We present results from simulations using 2 flavours of O(a)-improved Wilson
quarks whose masses are about 1/3 of the physical strange quark mass. We
present new data on the mass of the singlet pseudoscalar meson and evidence of
the onset of chiral logarithms in the pion decay constant. The previously
observed suppression of the topological susceptibility at lighter quark masses
is confirmed. We report on the performance of the hybrid Monte Carlo algorithm
at light quark masses.

<h4>Aims</h4>In studying aetiological interactions of genetic, epigenetic and environmental factors in normal and abnormal developments of the dentition, methods of measurement have often been limited to maximum mesio-distal and bucco-lingual crown diameters, obtained with hand-held calipers. While this approach has led to many important findings, there are potentially many other informative measurements that can be made to describe dental crown morphology. Advances in digital imaging and computer technology now offer the opportunity to define and measure new dental phenotypes in 3-D that have the potential to provide better anatomical discrimination and clearer insights into the underlying biological processes in dental development. Over recent years, image analysis in 2-D has proved to be a valuable addition to hand-measurement methods but a reliable and rapid 3-D method would increase greatly the morphological information obtainable from natural teeth and dental models. Additional measurements such as crown heights, surface contours, actual surface perimeters and areas, and tooth volumes would maximise our ability to discriminate between samples and to explore more deeply genetic and environmental contributions to observed variation. The research objectives were to investigate the limitations of existing methodologies and to develop and validate new methods for obtaining true 3-D measurements, including curvatures and volumes, in order to enhance discrimination to allow increased differentiation in studies of dental morphology and development. The validity of a new methodology for the 3-D measurement of teeth is compared against an established 2-D system. The intra- and inter-observer reliability of some additional measurements, made possible with a 3-D approach, are also tested.<h4>Methods and results</h4>From each of 20 study models, the permanent upper right lateral and upper left central incisors were separated and imaged independently by two operators using 2-D image analysis and a 3-D image analysis system. The mesio-distal (MD), labio-lingual (LL) and inciso-gingival (IG) dimensions were recorded using our 2-D system and the same projected variables were also recorded using a newly developed 3-D system for comparison. Values of Pearson's correlation coefficient between measurements obtained using the two techniques were significant at the 0.01 probability level for variables mesio-distal and incisal-gingival with labio-lingual significant at the 0.05 level for the upper left side only, confirming their comparability. For both 2-D and 3-D systems the intra- and inter-operator reliability was substantial or excellent for variables mesio-distal, labio-lingual, incisal-gingival actual and projected and actual surface area. The reliability was good for inter-operator reliability measurement of the labio-lingual dimension using 3-D.<h4>Conclusions</h4>We have developed a new 3-D laser scanning system that enables additional dental phenotypes to be defined. It has been validated against an established 2-D system and shown to provide measurements with excellent reliability, both within and between operators. This new approach provides exciting possibilities for exploring normal and abnormal variations in dental morphology and development applicable to research on genetic and environmental factors.

Gingival inflammation assessment by image analysis: measurement and validation.

<h4>Aim</h4>Gingival inflammation may be caused by injury or plaque-related diseases and reduction in inflammation can be a useful indicator of gingival recovery. There has been little research on development of non-index methods to measure gingival condition. The aims of the study were to investigate the reliability of the measurement of changes in gingival redness and swelling, using image analysis, and to compare this approach with an established method for assessing gingival overgrowth [J Clin Periodontol 28 (2001) 81].<h4>Method</h4>Twenty volunteers with gingival inflammation were recruited and digital images were taken. Duplicate measurements were made on the first visit by two examiners. At a subsequent visit following periodontal treatment, second images were taken. Gingival changes were determined by assessing redness and tooth surface area visible between the level of the inter-proximal papillae and the gingival margin. Tooth area measurements were compared with the established gingival overgrowth method.<h4>Results</h4>The method showed excellent reliability for both intra- and inter-examiner measurements of 0.968-0.998 and 0.769-0.947, respectively, according to the classification by Donner and Eliasziw of the Fleiss coefficient of reliability (repeat measures taken during the patients' first attendance). High correlation was found for gingival encroachment when compared with the established gingival overgrowth method.<h4>Conclusion</h4>This technique proved a reliable method for investigating changes in gingival redness. High correlation was found for gingival encroachment when compared with an established method.

One-loop renormalisation of quark bilinears for overlap fermions with
improved gauge actions

We compute lattice renormalisation constants of local bilinear quark
operators for overlap fermions and improved gauge actions. Among the actions we
consider are the Symanzik, L\"uscher-Weisz, Iwasaki and DBW2 gauge actions. The
results are given for a variety of $\rho$ parameters. We show how to apply mean
field (tadpole) improvement to overlap fermions. The question, what is a good
gauge action, is discussed from the perturbative point of view. Finally, we
show analytically that the gauge dependent part of the self-energy and the
amputated Green functions are independent of the lattice fermion
representation, using either Wilson or overlap fermions.
