Future Matters: Advance Care Planning Workshop

In the same way as it's good practice to have a will to protect our family and loved ones and make our financial wishes known, it’s also important to document our future health care wishes. We can do this in an Advance Care Plan.

Future Matters Islington, the free service run by Gentle Dusk in partnership with Age UK Islington, is here to help you put one in place.

What is an Advance Care Plan?

Advance care planning is the process of documenting your future health care wishes. Future Matters helps you to do this using Coordinate My Care, an online service from the NHS which records your wishes and treatment needs. It makes these visible to urgent care services including 111, out of hours GPs, the ambulance (in their vehicles) and the emergency departments.  This way everyone is in the loop, with the patient in the middle.

No one ever plans to have an accident, but documenting your personal health care and treatment wishes in advance can make all the difference if you are unable to communicate your wishes.

In thinking about this care, some decisions might feel very big (do you want to be resuscitated if you lose capacity?) and others might feel very small (favourite foods that cheer you up!) - but all of them matter.

A Coordinate My Care (‘myCMC’) plan provides recommendations for the urgent care services to follow in an emergency.

How can I get further support following the workshop?

Future Matters Islington offers free one-to-one support for making an advance care plan using the NHS Coordinate My Care service, so that your personal health care and treatment wishes are known.

Who is it for?

Future Matters Islington is available to all Islington residents or those with an Islington GP (with an interest in learning more about planning for their future. There are no age, health or financial criteria, planning ahead can help us and our loved ones feel less anxious and more in control at every stage.

Future Matters is a free service to help you plan for the future

In the London Borough of Islington , Future Matters is funded by North Central London CCG and run by Gentle Dusk in partnership with AgeUK Islington.

Future Matters raises awareness of the importance of end of life care planning and provides practical support to Islington residents and those with an Islington GP, planning their future. Future Matters provides free guidance and one-to-one support to help you and your loved ones put together end of life plans. These outline how you’d like your financial affairs to be managed, your healthcare wishes and where you’d like to be cared for. This is important in either an emergency, such as an accident, or in later life, when you might not have the capacity, or the time to make your wishes known.

Future Matters can help you to put in place Lasting Powers of Attorney and Advance Care Plans. It also offers a forum to discuss thoughts about death, dying and bereavement through regular events such as Death Cafes. Future Matters encourages us all to take time to think about and plan for how we would like to be cared for: in an emergency or in the future. This is important whatever age we are, as none of us knows what the future might hold.
