In order for a health and safety system to be robust and fit for purpose when an accident occurs there must be an investigation. The investigation must seek the root cause of the occurrence rather than focus on assigning blame.

In any organization's health and safety process accident investigations are intended to determine the cause of an incident, to identify unsafe conditions or acts and to recommend corrective actions so that similar incidents do not occur in the future.

This module will provide an understanding of the principles of accident investigation and the approaches taken to investigating accidents in the workplace.

An overview of the

  • key accident investigation techniques and the practicalities of information gathering
  • investigative interviewing and the preparation of accident reports
  • learning from accidents via the objective analysis of relevant case studies of past accidents
  • the identification of contributory causes of accidents via the application of causal analysis techniques.

On successful completion of this module students will be able to:

  • evaluate the key accident investigation techniques and the approaches to investigating workplace accidents.
  • apply the techniques to analyse real-life accident case studies and synthesize the related accident reports.
  • undertake meaningful investigations of workplace accidents.
  • prepare the related accident reports including the development of appropriate remedial / corrective actions to prevent similar repeat occurrences.
  • recognise and embrace the need for an effective approach to accident investigation that allows for the development of effective remedial / corrective actions to prevent similar repeat occurrences.

Applicants are normally expected to hold an NFQ level 8 primary honours degree (minimum H2.2), or equivalent and have at least 3 years of relevant industrial experience.

Entry requirements are established to ensure the learner can engage with the course material and assessments, at a level suitable to their needs, and the academic requirements of the module. By applying to this micro-credential, you are confirming that you have reviewed and understand any such requirements, and that you meet the eligibility criteria for admission.

Successful completion of this module does not automatically qualify you for entry into a further award. All programme applicants must meet the entry requirements listed if applying for a further award.

The fees for this programme are €1,000
